Residential Facilities for Groups are licensed by Nevada's Bureau of Health Care Quality and Compliance (HCQC). Applications for a new facility or a change of ownership are filed through the state's online licensing system with supporting documents including a Nevada business licence, evidence of zoning approval, insurance, a surety bond, ownership disclosure, a three-year business history and financial information, an administrator licence, background checks and a State Fire Marshal inspection. Facilities with more than ten beds also go through construction plan review.

How does Nevada define an RFG?
Under NRS 449.017, a residential facility for groups is an establishment that furnishes food, shelter, assistance and limited supervision to a person with an intellectual or physical disability or a person who is aged or infirm. The term includes assisted living facilities.
What documents does an application require?
HCQC's checklist for initial and change-of-ownership applications for this facility type lists the core items below. Fees are set in NAC 449.016.
- Notarized application and endorsement selection
- Application fee plus per-bed fee (NAC 449.016)
- Nevada State Business License from the Secretary of State
- Evidence of zoning approval from the city or county
- Certificate of insurance and an original surety bond
- Lease agreement or recorded deed showing the right to operate at the location
- Entity documents: articles, bylaws or operating agreement
- Three-year business history and financial status information (NAC 449.011)
- Administrator licence and three administrator references
- Criminal background checks, including corporate officers
- Certificate of compliance from the State Fire Marshal
Who must be disclosed as an owner?
NAC 449.011 requires full ownership information, including each natural person who owns the facility, holders of a direct or indirect interest of 10 percent or more who also hold secured obligations on the facility, and the officers and directors of a corporate applicant. Ownership changes while the application is pending must also be reported.
Which endorsements are available?
Endorsements include assisted living, Alzheimer's disease and related dementia, and population-specific endorsements. HCQC states that new applicants can no longer apply for a chronic illness endorsement: the regulation (NAC 449.2766) was repealed by R043-22.
For a sale this matters: a buyer applies in their own name, so a facility whose current licence still carries the chronic illness endorsement should not assume the buyer can obtain it.
Are larger facilities treated differently?
Yes. HCQC lists construction plan review, and a food establishment application for the kitchen, for residential facilities for groups with more than ten beds.
